How to make an appointment

Call the office at (239) 322-3817 to schedule an appointment with Dr.Alik.

After you’ve scheduled your appointment, please download and fill out the Patient Intake and Consent to Treatment forms to bring to your consultation.

Our HIPAA Policies can be downloaded for your personal use.

Our clinic does not participate in any insurance or Medicare plans. Please review our Terms and Conditions for more information.

Standard Policies and Procedures

If you need to cancel an appointment, please get in touch with the clinic as soon as you realize a conflict in your schedule has arisen. There is no fee to reschedule appointments with more than 24 hours notice. Appointments canceled within 24 hours may be subject to a $200 cancellation fee.
